PLEASE NOTE THAT IN ADDITION TO THE ABOVE COMPLETED DOCUMENTS A SCANNED COPY OF YOUR PASSPORT IS REQUIRED. IF YOU ARE NOT A U.S. CITIZEN A SCANNED COPY OF YOUR VISA OR ALIEN RESIDENCE CARD IS ALSO A REQUIREMENT. (COPY OF SUBMITTED APPLICATION FOR VISA IS ACCEPTABLE) IT IS ALSO A REQUIREMENT THAT ALL FAMILY MEMBERS LISTED ON THE 3299 ALSO PROVIDE COPIES OF THEIR PASSPORTS AND VISA'S IN THE SAME MANNER.
